This service includes:

• Individualized caregiver matching based on family needs, routines, and preferences

• Introduction to qualified caregivers for in-home support

• Guidance around routines, expectations, and transitions

• Follow-up support after initial match

• Assistance with rematching if needed

All placements are thoughtfully aligned with your family’s needs and stucture.

Investment is a one-time placement fee, determined after consultation. Optional premium add-ons are available to further enhance support.

Includes all components of Foundational Care Connection, with added focus on developmental needs and in depth support in key areas.

• Incorporation of behavioral informed strategies in collaboration with the child’s care team

This level includes all core service components, with added support for families who are connected to or searching for licensed therapy providers.

The focus is on serving as a resource to help families identify and connect with appropriate professionals, while supporting communication and consistency across settings.

Additional focus areas include:

• Providing guidance and resources to help families find therapy providers, schools, camps, and supportive environments tailored to their child’s needs.

• Supporting families in building a care team that aligns with their child’s unique needs and goals

• Maintaining a developing network of trusted providers to share as a resource (as available)

• Offering coordination support between caregivers and external approved providers only when requested and authorized by the family

• Sharing relevant observations or updates with providers, when appropriate and approved

• Supporting consistency across home, school, and therapy environments by reinforcing provider recommendations

Services do not include directing, modifying, or replacing the clinical work of licensed professionals.

Investment is customized based on the scope of coordination, number of providers, and level of ongoing integration within the home
